We are seeking a skilled Patient Care Specialist to join our team in providing high-quality patient care in home-based settings throughout the greater Columbia area.

Key Responsibilities:
  • Perform patient care duties such as obtaining vital signs and documenting patient care encounters.
  • Assist the Nurse Practitioner in performing lab tests, blood draws, wound care, and other procedures.
  • Conduct home visits to assist patients in connecting to a Provider via telehealth.
  • Perform administrative tasks as needed, including EMR experience.
Requirements:
  • Current CPR card
  • Current Drivers License
  • Clean Driving Record
  • Patient Care Experience Required

Applicants must be able to lift a minimum of 50 lbs., drive/ride in a company-owned car for long distances, and physically meet the demands of the job. Pay will be based upon experience.
