Job Summary

The Patient Advocate role at Proactive MD is a critical position that requires a unique blend of clinical knowledge, communication skills, and empathy. As a Patient Advocate, you will serve as a conduit of care between the health center and the community, ensuring seamless coordination of patient care and facilitating personalized, informative, and compassionate care.

Key Responsibilities
  • Coordination of Care: Under the direction of the Provider, you will coordinate care with external specialists, imaging centers, labs, medical equipment providers, counselors, and community resources.
  • Patient Engagement: Engage with patients to encourage use of the Health Center, coordinate follow-up visits, and develop outreach campaigns and educational resources.
  • Patient Education: Provide information to patients regarding available community services, coordinate patient care and treatment plans, and assist patients in understanding benefits available to them through their health insurance program.
  • Health Center Synergy: Coordinate with Clinical Operations to ensure smooth patient flow, engage with providers to optimize utilization of skills and abilities, and participate in Health Center-wide meetings to ensure excellent cross-communication and collaboration.
Requirements
  • 2+ years of experience working in a healthcare setting, medical or healthcare social work, or case management role.
  • Bachelor of Social Work Degree.
  • Current BLS certification through American Heart Association.
  • Medical terminology, including understanding of specialists.
  • Knowledge of the concepts and principles of medical case management.
  • Ability to communicate effectively and maintain working relationships with people from diverse backgrounds.
Preferred Qualifications
  • Master's degree in Social Work.
  • Knowledge of third-party payer requirements.
  • Licensure in LPCW, LCIS, LPC, or similar.
Work Environment and Physical Requirements

This role primarily operates in a professional office environment. The physical demands to perform the essential functions of this job include effective communication, mobility, ability to operate office equipment, and travel intermittently throughout the day.

Position Type and Expected Hours of Work

This role is expected to work a minimum of 40 hours/week as directed. Typical workdays are Monday through Friday, 8:00am to 5:00pm. This role is considered an exempt position, and evening and weekend work may occasionally be required as business needs dictate.
