Job Title: Patient-Centered Medical Home Care Coordinator

Job Summary:

The Patient-Centered Medical Home (PCMH) Care Coordinator plays a vital role in ensuring seamless care delivery and promoting effective communication among healthcare professionals. This position is responsible for coordinating care for an assigned patient population, ensuring that patients receive the necessary services and support to achieve optimal health outcomes.

Key Responsibilities:

  • Identify patients who require care coordination, including those with unmet clinical needs and quality measure gaps.
  • Utilize Epic registries and reports to identify patients and needs, and develop strategies to address these gaps.
  • Communicate effectively with patients, care teams, and providers to ensure continuity of care.
  • Schedule services and referrals as needed, and follow up to track data and ensure completion.
  • Maintain accurate records and data, and compile information for quality measures and projects.
  • Attend staff meetings and maintain open communication with providers and care team members.
  • Identify patient needs and barriers to care, and coordinate with community resources to address these issues.

Requirements:

Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN) with a strong commitment to patient-centered care. Must possess excellent communication and interpersonal skills, and be able to work independently under the supervision of the PCMH Clinical Manager.

Physical Requirements:

Ability to work in a fast-paced environment, with frequent travel to MUSCP-PC clinical locations. Must be able to lift, lower, push, and pull objects weighing up to 50 pounds, and work in a latex-safe environment.
