Responsibilities- Develop and implement individualized care plans to meet the unique needs of each patient
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication
- Conduct regular patient assessments and progress updates to inform treatment plans
- Provide education and support to patients and their families to promote independence and self-advocacy
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
- Masters or Bachelors in Social Work or related field
- Minimum 2 years experience in case management or social work in an inpatient setting
- Effective communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
- Competitive hourly rate: $37.00 - $45.00 USD
