Rehabilitation Case Manager

We are seeking a skilled Case Manager to join our interdisciplinary team at Virginia Mason Franciscan Health Rehabilitation Hospital. As a Case Manager, you will play a vital role in the care and recovery of our patients, working closely with healthcare professionals to develop and implement individualized treatment plans.
Responsibilities
- Complete departmental orientation and initial and annual competencies
- Collaborate with healthcare professionals to identify barriers to care and develop solutions
- Document patient progress and develop treatment plans in accordance with CMS guidelines
- Communicate with patients and caregivers to keep them informed of progress and provide information related to goal achievement and discharge plans
- Coordinate weekly patient care team conferences to facilitate development, monitoring, and refinement of treatment plans
- Review patient assigned CMG and identify potential missed comorbid conditions
- Other duties as assigned
Requirements
- Master's or Bachelor's degree in Social Work or as required by state
- Certification in Case Management or Rehabilitation Nursing preferred
- Minimum of 2 years social work or case management experience in an inpatient setting
- Effective oral and written communication skills in English, additional languages preferred
- Basic computer skills in Excel, Word, Outlook, PowerPoint, etc.

Salary range: $37.00 USD to $45.00 USD per hour. We would expect those hired into this job to be paid within this hourly rate, however, certain circumstances may allow us to pay outside of this hourly rate.
