Job Description

We are seeking an experienced Licensed Practical Nurse (LPN) to join our Hospice Care team. As an LPN, you will play a critical role in providing medical support to our patients and families, working collaboratively with our RNs and other healthcare professionals to deliver high-quality care.

Key Responsibilities
  • Provide clinical triage support to patients, families, and caregivers via telephone.
  • Review clinical documents, including medication records and referral information, to ensure accuracy and completeness.
  • Assist with the intake process, working closely with RNs and other team members to ensure timely intakes and efficient care delivery.
  • Support our field staff by providing phone support as needed and making patient visits when required.
Requirements and Qualifications
  • Graduate of an accredited LPN training program.
  • Current unencumbered LPN license in the State of Oregon.
  • Minimum 1 year experience in hospice, palliative care/oncology, geriatrics, and/or home health.
  • Experience providing triage support for patients.
  • CPR certification at hire or within 6 months in position.
Compensation and Benefits

The estimated hiring range for this position is $68,000 - $83,000 per year, with a 5% bonus target. We offer a comprehensive benefits package, including full benefits, and opportunities for career growth and development.
