Job Summary

We are seeking a highly skilled and compassionate Lead Health Care Professional to join our team as a Float Triage Nurse at Central City Concern. This is a full-time position that will require floating between our Old Town Clinic and Blackburn Primary Care Center locations.

About the Role

The Lead Health Care Professional (Float Triage Nurse) will be responsible for providing comprehensive community and clinic health services directed towards the prevention of disease, the promotion of health, and the client's self-care. The ideal candidate will have excellent communication skills, a strong ability to work independently, and a passion for delivering high-quality patient care.

Responsibilities
  • Provide direct care and delegate care of patients as indicated by patient needs.
  • Administer medications and treatments as directed by standing orders and/or by physician. Utilize Pyxis and maintain electronic bed list.
  • Observe and record pertinent information with regard to the care and condition of patients.
  • Formulate individualized nursing care plans and participate in patient care conferences.
  • Make referrals to hospitals and other agencies.
  • Provide patient care and other work-related directives for Subacute Technicians.
  • Assist the staff physician. Respond in emergency situations.
  • Give and receive report at shift changes.
  • Consult with other health team members to lend guidance as necessary.
Requirements

To be successful in this role, you will need:

  • A current RN license in the State of Oregon, as issued by the Board of Nursing.
  • Six months of nursing experience and/or recent educational preparation working with either psychiatric or chemical dependency populations.
  • Knowledge of nursing theory and practice, chemical dependency and treatment methods, application of knowledge in nutrition, sanitation, and personal hygiene.
  • Ability to follow verbal and written directions and to apply techniques for specific needs.
  • Knowledge of community resources, de-escalation methods, sufficient manual dexterity and physical ability to perform assigned tasks.
  • Ability to work in an environment where people may be hostile or abusive, manage time and meet deadlines, lift and move heavy objects.

Central City Concern offers a competitive salary range of $84,000-$110,000 per year, depending on experience, plus a rich benefits package including:
