Responsibilities
  • Assist residents in identifying resources and solutions to maintain their independence and satisfaction
  • Communicate with residents and families regarding support services available to meet their needs
  • Collaborate with healthcare providers to monitor and assess health conditions, safety issues, and resident needs
  • Attend Collaborative Care Meetings to determine risk potential of residents
  • Coordinate new resident orientation process to ensure a smooth transition into the community
Qualifications
  • Graduate from an accredited nursing school and a minimum of one to two years related experience required
  • Current State RN, LPN or LVN license
