About the Opportunity

As a Progressive Care Unit Rehabilitation Coordinator at the Detroit Medical Center, you will play a crucial role in coordinating patient care and rehabilitation services. We are seeking a highly skilled professional to join our team and contribute to the delivery of exceptional patient care.

Job Summary

The successful candidate will be responsible for developing and implementing individualized treatment plans for patients in the progressive care unit. This will involve collaborating with multidisciplinary teams, including physicians, therapists, and other healthcare professionals, to ensure that patients receive comprehensive and coordinated care.

In this role, you will also be responsible for conducting assessments, evaluating patient progress, and making recommendations for continued stay or discharge planning. You will work closely with families and caregivers to ensure that they are informed and engaged throughout the patient's care journey.

Responsibilities
  • Develop and implement individualized treatment plans for patients in the progressive care unit
  • Collaborate with multidisciplinary teams to ensure comprehensive and coordinated care
  • Conduct assessments and evaluations to inform treatment decisions
  • Maintain accurate records and documentation
  • Communicate effectively with patients, families, and caregivers
Requirements

To be successful in this role, you will need:

  • A Master's degree in Social Work, Speech Language Pathology, or a related clinical field
  • Certification in Case Management preferred
  • Three years of experience in a clinical setting
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
