Job Summary:The LPN Medication Care Manager is responsible for providing high-quality care and services to our residents by administering medication and treatments in a safe and organized manner. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Responsibilities:- Administer medication and treatments to residents in accordance with their individualized care plans.
- Review and update medication records to ensure accuracy and completeness.
- Collaborate with the Resident Care Director and Wellness Nurse to ensure that medication administration is carried out safely and effectively.
- Conduct regular audits to ensure that medication carts and treatment carts are properly stocked and maintained.
- Participate in the development and implementation of Individualized Service Plans (ISPs) and Individualized Care Plans (ICPs).
- Provide education and training to residents and their families on medication management and other health-related topics.
- Assist with the development and implementation of risk management programs and policies.
- Participate in regular meetings and training sessions to stay up-to-date on the latest developments in senior care and medication management.
- LPN or LVN licensure in the state of Ohio.
- Previous experience working with seniors and administering medication.
- Strong communication and interpersonal skills.
- Ability to work effectively in a fast-paced environment.
- Ability to maintain confidentiality and handle sensitive information.
- Ability to work flexible hours, including weekends and holidays.
