Job Summary

We are seeking a skilled and compassionate LPN to join our primary care team. As an Outpatient LPN, you will provide high-quality nursing care to patients and families in a variety of healthcare settings.

Key Responsibilities
  • Provide nursing care to patients and families along the health-illness continuum
  • Collaborate with the healthcare team to develop and implement individualized care plans
  • Delegate nursing tasks to unlicensed assistive personnel (UAPs) as needed
  • Communicate effectively with patients, families, and healthcare team members
  • Participate in quality improvement initiatives to enhance patient care and outcomes
Requirements
  • BLS - Basic Life Support - American Heart Association
  • LPN - Licensed Practical Nurse - National Council of State Boards of Nursing
  • Current license from the State of Ohio
  • Successful completion of Ohio Board of Nursing approved medication course
