Job Title: LPN Med Care Manager

At Sunrise Senior Living, we are committed to providing high-quality care and services to our residents. We are seeking a skilled LPN Medication Care Manager to join our team.

Responsibilities:
  • Administer medications and treatments in a safe and organized manner.
  • Document and initialize daily logs to track resident information.
  • Receive medication updates from the Resident Care Director or Wellness Nurse.
  • Ensure medications are passed according to schedule and document refusals or missed doses.
  • Maintain confidentiality of resident information and report concerns to the RCD or Wellness Nurse.
  • Restock medication carts and assist in checking medication.
  • Assess residents for "as needed" medication and document and report to supervisor.
  • Count narcotics with another Medication Care Manager or Lead Care Manager each shift.
  • Maintain and clean the Medication Room, medication carts, and treatment carts.
  • Follow re-fill process for medications.
  • Help residents maintain independence and promote dignity and physical safety.
  • Strive to understand and respond to each resident with empathy.
  • Audit medication carts and participate in daily Cross Over meetings.
  • Conduct Service and Health Updates as directed by the RCD.
  • Participate in the development of Individualized Service Plans (ISP)/Individualized Care Plans (ICP) and monthly updates.
  • Transcribe orders.
Requirements:
  • LPN, LVN, or state/province specific licensed nurse credential.
  • Maintain certifications as required by Sunrise and state/provincial regulations.
  • CPR and First Aid certification.
  • Ability to make choices and decisions and act in the resident's best interest.
  • Ability to react and remain calm in difficult situations.
  • Ability to handle multiple priorities.
  • Possess written and verbal skills for effective communication and level of understanding.
  • Demonstrate good judgment, problem-solving, and decision-making skills.
