As a member of our team, you will work collaboratively with primary care teams to ensure appropriate risk assessment for Medicare enrollees, leading to positive outcomes of care. Your responsibilities will include conducting onsite and telehealth Medicare Annual Wellness visits with adult primary care patients, updating patient information, reconciling medications, performing chronic condition screenings, and identifying health risks that require follow-up care. You will also promote healthy lifestyles, wellness education, and provide education about chronic conditions.
