Job Summary

The Southcentral Foundation (SCF) is seeking a highly skilled Case Management Support professional to join our team. As a key member of our Integrated Care Team, you will be responsible for coordinating and managing administrative duties to ensure seamless care delivery.

Key Responsibilities
  • Coordinate and manage administrative duties for Primary Care Providers and the wider Integrated Care Team
  • Ensure effective and efficient schedule coordination, verbal and written communication, information research and transfer, and system coordination
  • Work collaboratively with the core medical team to provide exceptional customer service and support
  • Develop and maintain strong relationships with customers and stakeholders
  • Stay up-to-date with changing policies, procedures, and regulations
Requirements
  • High School diploma or equivalent
  • 2+ years of administrative experience, preferably in a healthcare setting
  • Strong communication, organizational, and problem-solving skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
  • Knowledge of Alaska Native culture and community
Preferred Qualifications
  • 2+ years of Case Management Support experience
  • Demonstrated proficiency in administrative software and systems
  • Experience working with diverse populations and communities

As a Health Care Personnel (HCP) position, you will be required to have documentation of the following immunizations prior to your first day of employment: MMR, Varicella, Hepatitis B, Influenza, T-dap, and COVID-19 vaccination.
