Job Title: Spiritual Care Specialist

Ancora Home Health and Hospice LLC is seeking a compassionate Spiritual Care Specialist to provide emotional and spiritual support to patients and their families during end-of-life care.

Job Summary

The Spiritual Care Specialist will work with patients, families, and healthcare teams to address spiritual and emotional needs, providing counseling, education, and support. This role requires strong communication and interpersonal skills, as well as the ability to work independently and make judgments based on assessment of situations.

Responsibilities
  • Provide direct emotional and spiritual support to patients and families
  • Counsel patients and families as needed, taking into account their beliefs and values
  • Work with healthcare teams to enhance their sensitivity and educate them on the spiritual concerns of patients and families
  • Participate in IDG conferences to assess the spiritual needs of patients and families and report on services provided
  • Provide bereavement follow-up services as assigned
  • Maintain proper records of visits to patients and families
  • Make contact with clergy or other representatives of patients and families as indicated
  • Perform occasional liturgical assignments, such as memorial services
  • Conduct or make arrangements for funeral or memorial services as needed
  • Develop and maintain a resource group of clergies to whom specific aspects of spiritual care may be delegated
  • Arrange for on-call availability of spiritual services
  • Provide educational programs for community clergy, religious, and lay representatives as resources allow
  • Guide and educate family members and caregivers as needed
Requirements
  • Graduate of an accredited seminary or school of theology or appropriate certification in hospital or pastoral ministry
  • Minimum 2 years of experience as a chaplain or spiritual counselor, preferred
  • Ability to respond appropriately to a crisis outside of a hospital setting
  • Accept and adapt to different social, racial, cultural, and religious modes other than your own
  • Strong communication and interpersonal skills among different types of personalities
  • Ability to exercise initiative and independent judgment
  • Valid driver's license, clean driving record, and reliable automobile; proof of current automobile insurance
  • Criminal background check clearance/fingerprint clearance
  • Negative TB test required
  • Current CPR certification
Working Environment

The Spiritual Care Specialist will work indoors in agency offices and patient homes/care facilities, requiring travel to and from patients. The role involves exposure to elements, including odors, blood, body fluids, and excrements, as well as hazardous materials.
