We are seeking a highly skilled Registered Nurse to join our team at Yale New Haven Health. As a Psychiatric Nurse, you will play a critical role in providing exceptional care to our patients in the Transitional Youth Program.
Key Responsibilities- Provide High-Quality Patient Care: Utilize the nursing process to assess, diagnose, plan, implement, and evaluate patient care, ensuring the highest level of patient-centered care.
- Collaborate with Healthcare Team: Work effectively with the healthcare team to coordinate care delivery and transitions, promoting a safe and healthy environment for patients.
- Develop and Implement Care Plans: Develop and modify individualized care plans that prescribe strategies and alternatives to attain expected outcomes, ensuring patient-centered care.
- Communicate Effectively: Communicate effectively with patients, families, and healthcare team members, respecting patient values, preferences, and expressed needs.
- Participate in Quality Improvement: Contribute to quality nursing practice, evaluating and improving care processes to ensure safe, effective, and fiscally responsible care.
- Education: Graduation from an accredited school of nursing, with a Bachelor of Science in Nursing (BSN) preferred.
- Licensure: Current licensure as a Registered Nurse in the State of Connecticut, with clinical specialty certification highly encouraged.
- Experience: Previous experience as a Registered Nurse, with a strong foundation in psychiatric care and transitional youth services.
This role requires prolonged standing, walking, and use of sight, hearing, and touch. May be exposed to infectious diseases and physical, mental, and emotional illness, as well as end-of-life conditions and death. Moderate stress and risk of injury from patient care.
