The Patient Navigator will play a critical role in building strong relationships with HIV-positive patients, removing barriers to care by identifying resources and navigating health services. This is an exciting opportunity for a professional who is passionate about helping others.

**Job Summary**: Provide direct services under supervision, execute medical service plans, accompany patients to appointments, perform entitlement assessments, coordinate logistics, and maintain records.

**Requirements**: Associate's degree in human services or social work, 12 months experience in patient navigation, strong socio-cultural identification with the target population, fluency in one or more Asian languages preferred, excellent written and verbal communication skills in English.

**Compensation**: Estimated salary range $45,000 - $55,000 per year, based on experience and qualifications.
