RN Navigator Pulmonary Care Specialist

We are seeking a skilled RN Navigator to join our team at the University of Wisconsin Hospitals and Clinics Authority. As a RN Navigator, you will play a critical role in helping patients and families navigate the pulmonary care process. Your expertise will be invaluable in collaborating with multidisciplinary teams to ensure seamless care transitions and optimal patient outcomes.
Responsibilities:
* Work closely with pulmonary providers to develop and implement coordinated care plans for patients
* Serve as a conduit of information between inpatient and outpatient services, pre- and post-surgical care
* Participate in continuous quality improvement initiatives to enhance patient care
* Utilize strong organizational and multitasking skills to manage multiple priorities and deadlines

Requirements:
* Bachelor's degree in Nursing (BSN)
* Minimum three years of relevant clinical nursing experience
* Preferred one year of experience in pulmonary care
* Registered Nurse license in the State of Wisconsin and Basic Life Support/CPR certification
