Job Summary

We are seeking a highly organized and detail-oriented Referral Management Coordinator to join our team at Trios Health. As a key member of our healthcare team, you will be responsible for coordinating and managing patient referrals from initial contact through to the appointment with the specialist.

Key Responsibilities
  • Referral Management: Coordinate and manage patient referrals from initial contact through to the appointment with the specialist.
  • Ensure Accurate Documentation: Ensure all necessary documentation and patient information is accurately collected and submitted.
  • Track Progress: Track the progress of referrals and follow up as needed to ensure timely completion.
Communication
  • Primary Point of Contact: Serve as the primary point of contact for patients, healthcare providers, and specialists regarding referral status.
  • Effective Communication: Communicate effectively with patients to explain the referral process and provide updates.
  • Liaise with Healthcare Providers: Liaise with healthcare providers to clarify referral details and obtain additional information if required.
Data Management
  • Referral Records: Maintain and update referral records in the electronic health records (EHR) system.
  • Generate Reports: Generate reports on referral trends, volumes, and outcomes for management review.
  • Confidentiality and Security: Ensure confidentiality and security of patient information in compliance with HIPAA regulations.
Process Improvement
  • Identify Opportunities: Identify opportunities for improving the referral process and implement solutions to enhance efficiency and patient experience.
  • Develop and Update Policies: Develop and update referral policies and procedures as needed.
  • Provide Training: Provide training and support to staff on referral processes and best practices.
Leadership
  • Supervise and Mentor: Supervise and mentor a team of referral coordinators.
  • Assign Tasks: Assign tasks and monitor team performance to ensure high standards of service are maintained.
  • Conduct Team Meetings: Conduct regular team meetings to discuss progress, challenges, and areas for improvement.
Requirements
  • Excellent Organizational Skills: Excellent organizational and multitasking abilities.
  • Strong Communication Skills: Strong communication and interpersonal skills.
  • Proficiency in EHR Systems: Proficiency in using electronic health records (EHR) systems and other relevant software.
  • Knowledge of HIPAA Regulations: Knowledge of HIPAA regulations and patient confidentiality standards.
  • Ability to Work Independently: Ability to work independently and as part of a team.

This is a safety-sensitive position.

Salary: $25.10
