Job Summary

We are seeking a skilled Case Manager Social Worker to join our team at Northwestern Medicine. As a key member of our interdisciplinary team, you will play a vital role in ensuring the seamless transition of patients from admission to discharge.

Responsibilities
  • Act as the primary liaison between the interdisciplinary team and patients/caregivers, providing education and facilitating discharge coordination.
  • Follow patients from admission to discharge, communicating with external payers and attending physicians to monitor utilization and length of stay.
  • Work with patients/caregivers to plan and facilitate timely and appropriate discharge plans, ensuring a safe transition to the next level of care.
  • Assess and address the needs of patients, providing referrals to community resources and coordinating with patient/financial services and admissions to ensure smooth transitions between levels of care.
  • Provide ongoing education to the interdisciplinary team on patients' insurance benefits and status of length of stay approval.
  • Request authorization for continued stay from payers as recommended by the interdisciplinary team, coordinating appeals on any concurrent denials.
  • Monitor and communicate resource utilization with attending physicians, identifying variances from established benchmarks on cost, outcome, and length of stay.
Qualifications

Required:

  • Master's Degree in Social Work with current LSW or LCSW license in the state of Illinois.

Preferred:

  • Previous experience with Case Management and/or Discharge Planning in a hospital setting.
  • Previous experience in a rehabilitation setting.
