Job Summary

We are seeking a highly motivated and experienced Community Support Specialist to join our Adult Outpatient Services team at the DuPage County Health Department. As a key member of our team, you will provide essential support to individuals with serious mental illness, promoting their recovery and well-being through individual and group counseling, community support, and psychiatric treatment.

Responsibilities
  • Manage a caseload and maintain regular contact with clients outside of the office, home-based, and community-based to meet client needs and engage at-risk clients.
  • Act as a client advocate to facilitate access to resources and services.
  • Work collaboratively with providers to achieve client goals.
  • Develop and maintain client treatment plans.
  • Provide intervention to support client self-management of their illness.
  • Demonstrate the ability to engage adults and families.
  • Act as a liaison between healthcare resources and clients.
  • Participate in client staffing and team meetings.
  • Demonstrate independent judgment and discretion in making position-related decisions.
  • Meet established productivity standards.
  • Perform concurrent documentation.
  • Maintain collaborative relationships with co-workers to effectively support clients.
  • Participate in emergency activities as assigned.
  • Maintain required training, licensure, and/or certification.
  • Maintain confidentiality of privileged information and adhere to client privacy laws.
  • Demonstrate sensitivity and understanding of other ethnic groups and cultures.
  • Adhere to department guidelines for attendance and punctuality.
  • Provide skill-building and case management supports to clients in the community.
Requirements
  • Completion of a Bachelor's Degree in Psychology, Social Work, or a related field, and one year of behavioral health experience; or an equivalent combination of training and experience.
  • Designated positions may be required to possess a valid Illinois Driver's License and a motor vehicle background check.
