We are seeking a highly motivated and experienced Community Support Specialist to join our Adult Outpatient Services team at the DuPage County Health Department. As a key member of our team, you will provide essential support to individuals with serious mental illness, promoting their recovery and well-being through individual and group counseling, community support, and psychiatric treatment.
Responsibilities- Manage a caseload and maintain regular contact with clients outside of the office, home-based, and community-based to meet client needs and engage at-risk clients.
- Act as a client advocate to facilitate access to resources and services.
- Work collaboratively with providers to achieve client goals.
- Develop and maintain client treatment plans.
- Provide intervention to support client self-management of their illness.
- Demonstrate the ability to engage adults and families.
- Act as a liaison between healthcare resources and clients.
- Participate in client staffing and team meetings.
- Demonstrate independent judgment and discretion in making position-related decisions.
- Meet established productivity standards.
- Perform concurrent documentation.
- Maintain collaborative relationships with co-workers to effectively support clients.
- Participate in emergency activities as assigned.
- Maintain required training, licensure, and/or certification.
- Maintain confidentiality of privileged information and adhere to client privacy laws.
- Demonstrate sensitivity and understanding of other ethnic groups and cultures.
- Adhere to department guidelines for attendance and punctuality.
- Provide skill-building and case management supports to clients in the community.
- Completion of a Bachelor's Degree in Psychology, Social Work, or a related field, and one year of behavioral health experience; or an equivalent combination of training and experience.
- Designated positions may be required to possess a valid Illinois Driver's License and a motor vehicle background check.
