Monday through Friday at Waukesha Memorial Hospital, with occasional rotations to Oconomowoc Memorial Hospital and Mukwonago Hospital.

Your Responsibilities:
The Senior Nutrition Specialist collaborates with healthcare team members to deliver medical nutrition therapy by assessing the nutrient needs based on age and specific health conditions of patients, thereby creating and executing the patient's nutrition care plan.

Key Duties:

Gathers patient health information, prioritized according to the patient's condition or needs, employing suitable assessment methods while involving the patient, family, and other healthcare professionals as necessary.

Formulates a care plan based on identified issues, recommending interventions to achieve desired outcomes. Adjusts priorities, outcomes, and interventions in collaboration with the patient, family, and other healthcare providers. Ensures systematic, ongoing data collection with appropriate documentation. Analyzes assessment data to determine immediate and long-term patient care requirements. Identifies personalized expected outcomes in partnership with the patient, family, and other healthcare providers. Continuously assesses the patient's progress towards achieving outcomes. Communicates and documents screenings, dietary history, and the patient's response to dietary consultations as required by Clinical Practice Model (CPM). Acquires and consistently enhances knowledge in contemporary dietetic practices. Acts as a mentor to dietetic interns at ProHealth Care facilities. Must be capable of fulfilling work responsibilities at all ProHealth Care locations.

Qualifications:
Registered Dietitian (RD, RDN)

Bachelor's Degree with a minimum of 2 years of experience as a Registered Dietitian and DCES (Certified Diabetes Care & Education Specialist) certification. Why Choose ProHealth Care:
