Key responsibilities include:
- Identifying resources and support for Case Management Coordinators.
- Facilitating communication with administrative personnel.
- Coordinating team meetings to foster collaboration.
- Expanding the capabilities of the Case Management department.
Essential Qualifications: Familiarity with the CalAim initiative and ECM, along with a minimum of six months of experience in the ECM initiative is required.
Note: Job responsibilities may evolve over time.
