Position Overview

The Case Management Team Leader plays a crucial role in ensuring optimal outcomes for the Case Management division. This position entails overseeing and enhancing care plans, evaluations, and staff interactions to cater to the varied needs of our clientele.

The Case Management Team Leader will also manage a caseload of clients, working closely with Case Management Coordinators and engaging directly with clients, their families, and healthcare professionals.

Key responsibilities include:

  • Identifying resources and support for Case Management Coordinators.
  • Facilitating communication with administrative personnel.
  • Coordinating team meetings to foster collaboration.
  • Expanding the capabilities of the Case Management department.

Essential Qualifications: Familiarity with the CalAim initiative and ECM, along with a minimum of six months of experience in the ECM initiative is required.

Note: Job responsibilities may evolve over time.
