Job Summary:

The LCSW/LMFT Transitional Care Case Manager will be responsible for providing services to Medi-Cal members transitioning to and from hospitals, institutions, and skilled nursing facilities to home or community-based settings. This role requires strong communication and coordination skills, as well as the ability to work effectively with various stakeholders.

Responsibilities:
  • Provide services to Medi-Cal members transitioning to and from hospitals, institutions, and skilled nursing facilities to home or community-based settings.
  • Assist with coordination of care, provided resource linkages, and collaborate with other team members.
  • Shift: Monday to Friday, 8am to 5pm/ Work Location: 2-3 days Onsite in Rancho Cucamonga, CA and 2-3 days from home.
Requirements:
  • Master of Social Work and active CA LCSW or LMFT License.
  • 3-5 years of behavioral health experience.
  • Experience in motivational interviewing and/or other evidenced-based communication strategies.
Preferred Qualifications:
  • MCO and/or Case Management experience.
  • Bilingual in Spanish.
