About the Role

We are seeking a highly skilled and experienced Case Manager to join our team at Blue Shield of California. As a Case Manager, you will play a critical role in ensuring that our members receive high-quality, cost-effective care.

Key Responsibilities
  • Develop and implement individualized care plans that promote quality, access, and cost-effective outcomes.
  • Collaborate with healthcare providers, members, and other stakeholders to ensure seamless care coordination.
  • Conduct thorough assessments of members' health needs and develop targeted interventions to address identified gaps in care.
  • Provide education and support to members to promote healthy behaviors and self-management of chronic conditions.
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed.
Requirements
  • Current CA RN License
  • Bachelor's degree in Nursing or advanced degree preferred
  • Certified Case Manager (CCM) Certification or in process of completing certification
  • At least 5 years of prior experience in nursing, healthcare, or related field
  • At least 3 years of managed care experience preferred
