Job Summary

Radias Health is seeking a skilled Mental Health Practitioner to join our Community Care Service Team (CCST). This role specializes in supporting individuals with high unmet needs and would benefit from additional support. The position is hybrid, allowing for work from home when not meeting with individuals in the community.

Key Responsibilities
  • Develop, write, implement, evaluate, and revise treatment goals and plans in collaboration with the client and the treatment team.
  • Assist individuals in establishing additional community supports and communicate with natural supports.
  • Coordinate services with other providers, including local community hospitals, Mental Health Case Management, outpatient behavioral health providers, lawyers, guardians, care coordinators, and probation officers.
  • Develop individualized crisis plans coordinated with community emergency support services.
  • Meet with clients to explain CCST services, assess client needs, establish and implement a care plan to address those needs, and arrange for resources necessary for community transition.
  • Assist clients with all aspects of psychiatric rehabilitation and obtaining and maintaining housing.
  • Teach clients independent living skills, such as cooking, shopping, cleaning, personal hygiene, transportation, and budgeting.
  • Attend commitment and other court hearings as needed and complete required court reports.
  • File reports with Vulnerable Adult, Child Protection, the Ombudsman's Office, and other mandated/required reports when applicable.
  • Actively engage and reach out to clients' family members/significant others, including landlords, employers, and other treatment team members after obtaining client permission.
Requirements
  • Bachelor's degree in behavioral sciences or related field (if no degree, 30 semester credit hours in behavioral health sciences) and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders - OR-
  • Masters or other graduate degree in behavioral health sciences or related field -OR-
  • If no degree, must have 4,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders
  • Must have a car, auto insurance, and driver's license.
