Physician - Community Health Specialist

About the Role:
Minnesota Community Care is seeking a dedicated and compassionate Physician to join our team in the St. Paul/Minneapolis area. As a Family Medicine Physician, you will be responsible for providing high-quality patient care to our diverse patient population.
Responsibilities:
- Provide comprehensive primary care to patients of all ages, including routine check-ups, health screenings, and treatment of acute and chronic conditions.
- Develop and implement patient care plans that address the physical, emotional, and social needs of our patients.
- Collaborate with other healthcare professionals to ensure seamless care coordination and referrals.
- Stay up-to-date with the latest medical research, guidelines, and best practices to provide evidence-based care.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
- Medical school diploma and DEA certification.
- Minnesota state medical license and active BLS certification.
- Minimum four years of post-residency experience in a community-based setting.
- Bilingual Spanish/Hmong language skills highly preferred.
- Proof of COVID vaccination required, with religious and medical exemptions considered.
