Administrative Care Coordinator Specialist

Key Responsibilities
- Meet with community members to assess their needs and develop personalized plans to address those needs
- Identify gaps in service and work with clients to fill those gaps
- Research and implement new programs to improve community wellness and quality of life
- Ensure regulatory compliance and maintain accurate records
Requirements
- Bachelor's Degree or higher in social work or a related field
- At least 2 years of experience providing direct care services to individuals with disabilities
- Strong communication and interpersonal skills
- Experience with basic computer programs, such as Microsoft Office suite
