About the Position:

This position requires a highly skilled and organized individual who can effectively coordinate the delivery of patient care in the hospital setting. As a Hospital Care Coordination Expert, you will be responsible for ensuring that patients receive seamless care from admission to discharge.

Key Responsibilities:

  • Coordinate patient care in the hospital setting, including assessment, planning, implementation, and evaluation
  • Collaborate with healthcare providers to develop and implement individualized care plans
  • Provide education to patients and their families on disease management and treatment options
  • Document patient information and care activities accurately and timely

Requirements:

  • Graduate of accredited school of nursing with 2-3 years' experience in hospital, critical care, or home health nursing
  • Pediatric experience preferred
  • CPR certification/PALS certification
  • Texas RN license
  • Texas drivers license in good standing
