Job Summary: We are seeking a dedicated Pediatric Nursing Care Coordinator to join our team at Cook Children's Health Care System. As a key member of our healthcare team, you will play a vital role in providing exceptional care to our patients and their families.

About the Role: The Pediatric Nursing Care Coordinator will be responsible for coordinating services to meet the patients' care needs in a field setting. This includes documenting and providing information necessary for clinical, regulatory, and financial management of the patient. You will work closely with multidisciplinary teams to ensure seamless patient care and outcomes.

Responsibilities:

  • Coordinate care services in a field setting to meet patient needs.
  • Document and provide information necessary for patient care management.
  • Collaborate with multidisciplinary teams to ensure patient-centered care.
  • Communicate effectively with patients, families, and healthcare providers.
  • Maintain accurate records and reports.

Requirements:

  • Current RN license in Texas.
  • Current CPR certification/PALS certification.
  • 2-3 years' experience in hospital, critical care, or home health nursing.
  • Pediatric experience preferred.
