Geriatric Emergency Medicine Nurse Practitioner Coordinator

We are seeking an experienced Nurse Practitioner to join our team as an Administrative Nurse Practitioner Coordinator in Geriatric Emergency Medicine. This role will provide direct clinical care to geriatric patients in the Emergency Department and lead implementation efforts for the Geriatric Emergency Department.
Key Responsibilities- Provide direct clinical care to geriatric patients in the Emergency Department, including performing health histories, geriatric screenings, and assessments.
- Lead implementation efforts for the Geriatric Emergency Department, including developing and implementing program standards and goals.
- Collaborate with interdisciplinary teams to ensure seamless transitions of care across settings.
- Develop and implement educational programs for staff and patients to promote healthy adjustment to changes in health conditions.
- Participate in quality improvement activities and protocols to improve patient safety and outcomes.
- Current registration as a Professional Nurse in New York and registration as a Nurse Practitioner in New York.
- Master's Degree in Nursing and certification as a Nurse Practitioner.
- 5 years of experience as an advanced practice nurse.
- Excellent written and verbal communication skills.
- Ability to develop, implement, and sustain clinical programs.
