Geriatric Emergency Medicine Nurse Practitioner Coordinator

NYU Grossman School of Medicine is seeking a highly skilled Nurse Practitioner to join our team in the Geriatric Emergency Department. As a key member of our interdisciplinary team, you will provide direct clinical care to older adult patients, lead implementation efforts, and participate in care coordination across settings.
Key Responsibilities- Provide comprehensive care to older adult patients, including health history, geriatric screening, and assessments.
- Ensure seamless transitions of care through collaborative communication with care providers.
- Develop and implement plans of care that optimize patient outcomes, considering patient and family preferences.
- Prescribe medications, order laboratory and diagnostic studies as indicated.
- Facilitate physician, APP, and nurse collaboration and interdisciplinary team building.
- Function within scope of practice and delineation of privileges as an Adult or Family Nurse Practitioner.
- Collaborate in the design, development, and facilitation of educational offerings for staff performance development.
- Act as a professional role model to all levels of staff.
- Current registration as a Professional Nurse in New York and registration as a Nurse Practitioner in New York.
- Master's Degree in Nursing.
- Certification required (as soon as feasible following graduation from NP graduate program) by ANCC or Board Certification from other specialty organization that certifies Nurse Practitioners in a clinical area of practice.
- 5 years of experience as an advanced practice nurse.
- Evidence of ability to manage data and use of computer applications.
- Excellent written and verbal communication skills.
- Demonstrated ability to interface effectively with other disciplines and outside agencies to achieve improvement in patient outcomes, lower length of stay, and efficient resource utilization.
