Geriatric Emergency Medicine Nurse Practitioner Coordinator

We are seeking an experienced Nurse Practitioner to join our team in the Geriatric Emergency Department at NYU Langone Health. The successful candidate will provide direct clinical care to older adult patients in the Emergency Department, while also leading implementation efforts and participating in care coordination across settings.
Key Responsibilities- Provide comprehensive care to older adult patients in the Emergency Department, including health history, geriatric screening, and physical assessments.
- Ensure seamless transitions of care across settings through collaborative communication with care providers.
- Develop and implement plans of care that optimize patient outcomes, considering patient and family preferences.
- Collaborate with interdisciplinary teams to evaluate patient-specific plans of care and implement best practices.
- Prescribe medications, order laboratory and radiological studies as indicated, and facilitate physician/NP/nurse collaboration.
- Function within scope of practice and delineation of privileges as an Adult or Family Nurse Practitioner.
- Current registration as a Professional Nurse in New York and registration as a Nurse Practitioner in New York.
- Master's Degree in Nursing and certification as a Nurse Practitioner.
- 5 years of experience as an Advanced Practice Nurse.
- Excellent written and verbal communication skills, with ability to interface effectively with other disciplines and outside agencies.
- Ability to develop, implement, and sustain clinical programs, adapt to change, and facilitate group processes.
