Position Overview:

The Geriatric Care Nurse Practitioner plays a vital role in delivering exceptional healthcare services to senior residents across various healthcare environments, including skilled nursing facilities, assisted living, and independent living communities.

Key Responsibilities:
As a Nurse Practitioner, you will be responsible for:

  • Conducting comprehensive assessments and managing both chronic and acute health conditions.
  • Delivering health education to residents and their families.
  • Formulating and communicating effective treatment plans to residents and relevant healthcare providers.
  • Ordering and interpreting diagnostic tests, adjusting treatment strategies as necessary.
  • Maintaining accurate and compliant documentation within our electronic medical record system.
  • Collaborating with local and regional clinical leadership to meet organizational goals.
  • Supporting initiatives aimed at enhancing value-based care.
  • Ensuring a positive customer experience to promote high satisfaction and retention rates.
  • Practicing in accordance with professional standards and regulatory requirements.
  • Coordinating care with other healthcare providers, including telemedicine specialists.
  • Participating in family meetings and providing advanced care planning when required.

Qualifications:
Required:
- Master of Science in Nursing, Doctor of Nursing Practice, or equivalent NP certification.
- National certification from AANP or ANCC.
- Active and unrestricted state licensure.
- Active DEA registration.
- Authorization to work in the United States.
- Minimum of 2 years of clinical experience in a geriatric, adult, or family practice setting.
- Flexibility to adapt to varying work environments and schedules.
- A strong commitment to providing healthcare to senior and low-income populations.

Preferred:
- Experience or training in geriatrics.
- Over 3 years of clinical experience as a Nurse Practitioner.
- Familiarity with coding, ICD-10, and HCC models.
- Background in metrics-driven healthcare practices.
- Openness to professional development and training opportunities.

Skills and Abilities:
- Excellent verbal and written communication skills.
- Detail-oriented with the ability to work independently and collaboratively.
- Strong customer service orientation with residents and team members.
- Effective time management skills, capable of prioritizing tasks to meet deadlines.
