Under the supervision of the Service Line or specialty program leader (such as Cardiovascular, Neuroscience, Cancer, GI, Orthopedic, Geriatric), the Patient Care Coordinator acts as the primary liaison for patients throughout their healthcare journey; from diagnosis and treatment planning to pre and post-operative care, discharge, and follow-up.

The Patient Care Coordinator serves as a vital resource for patients, actively seeking their input in the care plan.

This role involves collaboration with all relevant healthcare providers (physicians and the entire care team) to ensure a comprehensive plan of care while advocating for the patient's needs.

The Patient Care Coordinator provides education to patients and their families, facilitating access to hospital and community resources, as well as promoting community health education and screenings.


KEY RESPONSIBILITIES:

As the main advocate for the patient following their initial diagnosis, ensures that patients and their families fully comprehend the diagnoses, treatment strategies, and available resources until they are equipped to manage their care independently.

Empowers patients and families to express their concerns, preferences, and priorities to their healthcare team.

Facilitates patient entry into the program and coordinates their ongoing care.

Maintains oversight of the patient throughout the continuum of care services, guiding both the patient and care team through various transitions.

In collaboration with the patient's physician(s) and care team, develops a coordinated care plan that includes goals, interventions, and treatment strategies tailored to the patient's unique needs, preferences, beliefs, and values.

Coordinates resources according to patient requirements, insurance coverage, and community programs.

Identifies potential gaps in care or services, as well as issues with procedures or policies, and collaborates with the care team to implement improvements.

Assesses the educational needs of patients and caregivers, effectively translating and communicating clinical information. Provides clinical education regarding diagnoses, treatments, side effects, and post-treatment care.

Offers essential education and information on available options, facilitating informed shared decision-making and realistic goal-setting to empower patients and caregivers in their care journey.

Coordinates patient care and acts as a crucial link between the care team,
communicating with the multidisciplinary team (physicians, nurses, therapists, social workers, etc.) as necessary on behalf of the patient. Facilitates patient appointments and communication with physicians and support services (Social Work, PT, etc.). Identifies and addresses clinical and service delivery barriers to care. Coordinates multidisciplinary care conferences as required.

Engages in community education and outreach, including hosting informational events, community screenings, and marketing initiatives. Conducts ongoing community needs assessments to address healthcare disparities and barriers to care for patients.

Participates in quality improvement initiatives, collects data on performance measures, understands and utilizes the Performance Improvement process, and makes recommendations for process enhancement.

Takes responsibility for personal professional development by attending continuing education courses, seminars, and reviewing professional literature.

Adheres to all standards, policies, and procedures set forth by Northshore.
REQUIRED SKILLS AND QUALIFICATIONS:

  • Exceptional interpersonal skills, including conflict resolution abilities necessary for effective collaboration with patients, hospital staff, and physicians. Capacity to demonstrate empathy and sensitivity.
  • Strong written and verbal communication skills required for conducting educational sessions, community programs, and developing policies and educational materials.
  • Organizational skills essential for independent work with minimal supervision.
  • Proficient computer skills necessary to fulfill job responsibilities.
  • Able to manage and monitor multiple simultaneous projects effectively.
KNOWLEDGE, EXPERIENCE, AND LICENSURE/

REGISTRATION REQUIRED:
(Minimum qualifications to perform the job).

  • Registered Nurse with a Bachelor of Science in Nursing or completion of an accredited Physician Assistant program.
  • Current licensure to practice nursing or a valid Physician Assistant license.
  • A minimum of three (3) years of nursing or Physician Assistant experience in the relevant specialty field.
