About the Role:
CenterWell Senior Primary Care is seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a key member of our integrated care team, you will work closely with patients, physicians, and other healthcare professionals to provide high-quality, patient-centered care.
Responsibilities:
- Provide health promotion and maintenance through the diagnosis and treatment of acute illness and chronic conditions.
- Prescribe medication, examine patients, diagnose illnesses, and provide treatment.
- Work independently, exercising judgment and decision-making skills on complex issues.
- Collaborate with other healthcare professionals to develop and implement effective care plans.
Requirements:
- Bachelor's degree in a related field.
- 8 or more years of technical experience, including 2 or more years of project leadership experience.
- Active, unrestricted Nurse Practitioner/PA license in the appropriate state.
Preferred Qualifications:
- Masters in Science of Nursing/PA.
