Community Support Specialist

Pathlight Group is seeking a compassionate and dedicated Community Companion to join our team. As a Community Companion, you will have the opportunity to make a meaningful difference in the lives of individuals with disabilities.
Key Responsibilities:
- Engage and assist individuals in social and recreational activities, promoting independence and social skills.
- Develop and implement individualized plans to support the goals and needs of each person.
- Provide transportation and accompany individuals to appointments, events, and activities.
- Collaborate with care providers, family members, and other support staff to ensure seamless support.
- Document and report on individual progress, identifying areas for improvement and celebrating successes.
Requirements:
- High school diploma or equivalent.
- 2 years of experience working with individuals with disabilities.
- Valid driver's license and reliable transportation.
- First Aid/CPR certification.
