Geriatric Care Nurse Practitioner or Physician Assistant

About the Role
Hartford HealthCare (HHC) is seeking a full-time, experienced Nurse Practitioner or Physician Assistant for our REACH Program serving geriatric patients in the greater Fairfield area.
Key Responsibilities:
- Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
- Develop plans for further evaluation of health needs, including referrals to other health team members or specialists.
- Provide appropriate treatment for problems within the scope of the provider's practice as designated by protocols and identify problems outside of their scope of practice and refer appropriately to physician.
Requirements:
- Unrestricted State of Connecticut Medical License (or eligible)
- APRN (Family, Adult/Geri Nurse Practitioner) or PA training
- At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA)
