Job Description

American Family Care Bridgeport seeks a highly skilled Advanced Practice Provider to join our team. As a key member of our primary care team, you will provide high-quality patient care, work collaboratively with other providers and staff, and contribute to the overall success of our practice.

Responsibilities:
  • Conduct thorough health histories, review patient records, and develop comprehensive medical assessments.
  • Formulate medical and nursing diagnoses, and institute therapy or referrals as needed.
  • Provide emergency care and stabilization measures as required.
  • Interpret and analyze patient data to determine patient status and treatment.
  • Offer health care guidance and education to patients and their families.
Qualifications:
  • Excellent communication and interpersonal skills.
  • Professional appearance and attitude.
  • ANCC, AANP, or NCCPA Board Certification, with 1 year of ER/UC experience preferred.
  • Active and current Physician Assistant or Nurse Practitioner licensure in the state of practice, DEA number, and state-controlled substance certificate, as required.
  • Able to work independently and multitask effectively.
