Hartford HealthCare Medical Group Specialists is seeking a full-time, experienced Nurse Practitioner or Physician Assistant for our REACH (Resource for Elderly And Care at Home) Program serving our geriatric patients in the greater Fairfield area.

Role and Responsibilities:

  • Perform routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients.
  • Develop plans for further evaluation of health needs, e.g., referral to other health team members or specialists.
  • Provide appropriate treatment for problems within the scope of the provider's practice as designated by protocols and identifies problems outside of their scope of practice and refers appropriately to physician.

Requirements:

  • Licensure: Unrestricted State of Connecticut Medical License (or eligible)
  • Training: APRN (Family, Adult/Geri Nurse Practitioner) or PA
  • Experience: At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA)
