Job Responsibilities

The Program Manager (Chronic Care) will be responsible for the following:

  • Developing and implementing effective strategies to improve health outcomes for patients with chronic diseases
  • Oversight of the non-clinical aspects of our Chronic Care programs, including grant writing, program evaluation, and budget management
  • Collaboration with our multidisciplinary team to ensure seamless coordination of care and optimal patient outcomes
  • Grant writing and fundraising to support program initiatives
  • Program evaluation and quality improvement to ensure optimal patient outcomes
