We're seeking a highly skilled Health Coach to join our team at one of our offices in Hyannis, MA. As a key member of our primary care team, you will be responsible for establishing trusting relationships with patients and their families, advocating for their needs, and facilitating better health outcomes.
Salary and Benefits
The base pay for this full-time position is $24.25 per hour, based on a 40-hour workweek. You'll also enjoy a range of benefits, including paid sabbatical, employee assistance program, competitive medical, dental, and vision plans, and free One Medical memberships for yourself and your loved ones.
Job Description
Responsibilities:
- Capture relevant patient information while rooming patients, taking vitals, administering point-of-care testing, and performing age-appropriate screening assessments
- Assist with coordinating post-visit care by scheduling appointments with specialists, coordinating referrals, and sharing information with the patient's internal and external care team
- Collaborate with providers to monitor patients' health and ensure they receive necessary preventive measures
- Participate in daily practice operations, such as answering phone calls, responding to emails, and maintaining the clinical space
- Provide patient education on chronic disease management and coach patients using motivational interviewing techniques
- Offer anticipatory guidance to families with children, focusing on building healing relationships and promoting overall well-being
Requirements and Qualifications:
- At least one year of experience in a high-touch customer service or patient-facing role in a healthcare setting required
- Strong written and verbal communication skills
- Experience working on diverse, collaborative teams and adapting to change
- A proven track record of persistence, forward thinking, and proactive problem-solving under pressure
- Proficiency in computer technology, including typing, navigating the internet, and using multiple software systems simultaneously
