Key Responsibilities:
- Coordinate and schedule quality and qualified field employees with clients to ensure seamless care delivery.
- Maintain current client services and intake of new clients to agency, ensuring timely and efficient care coordination.
- Communicate and update all client care information into agency EMR system to ensure clients' Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs) needs are met.
- Supervise field caregivers and perform annual performance reviews to promote professional growth and development.
- Manage client/contract satisfaction with the highest level of customer service standards, communication, and documentation.
- Ensure all forms of communication with clients, families, health plan providers, and caregivers are documented timely to promote continuity of care across all interdepartmental functions.
- Responsible for generating client care plan assessments and system updates.
- Conduct audit review of caregiver's daily documentation logs to ensure compliance with client's ADL and IADL needs.
- Build relationships with clients to encourage compliance with care plans and client safety.
- Orientate and supervise field caregivers to client care plans and provide remedial training and mentoring as needed.
- Assess caregivers' compliance and understanding to client care plans on a quarterly basis or as needed based on client's level of care and changes in conditions in between quarterly visits.
- Work with Recruitment and Compliance departments to communicate recruitment needs and ensure active caregivers are in compliance with state and federal annual health and training requirements.
- Manage time and attendance of caregivers and ensure daily compliance with Electronic Visit Verification (EVV) requirements for timely payroll and billing processing.
- Run reports and utilize data from EMR and Electronic File Storage System to manage KPIs.
- Follow Agency Policies and Procedures
- Ensure all HIPAA regulations are adhered to.
- All other duties as assigned.
Requirements:
- College degree preferred or equivalent work experience.
- Travel will be required for business purposes. Employees must have a valid driver's license issued by the state in which they work and a satisfactory driving record.
- A specific understanding of geographical/cultural requirements of branch office is preferred.
- Ability to use computerized systems.
- Satisfactory verbal and written communication skills.
- Ability to work well under pressure.
Additional Requirements:
Bi-lingual – English/Spanish a plus.
