Job Summary

We're seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our Home Health team. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to deliver exceptional care.


Responsibilities:

  • Assess, plan, implement, and evaluate patient care needs and outcomes.
  • Provide skilled nursing services to patients in their homes, under the direction and supervision of a Registered Nurse.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Conduct thorough patient assessments, identifying and addressing physical, emotional, and social needs.
  • Develop and implement effective care plans, ensuring patient safety and well-being.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient care and progress accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.


Requirements:

  • Graduate of an accredited practical nursing program.
  • Current LPN license in the state of practice.
  • CPR certification.
  • One year of experience in a related field, preferably in home health or hospice care.
