Licensed Practical Nurse LPN Hospice Full Time

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Signature Healthcare at Home. As a Hospice LPN, you will provide high-quality patient care and support to individuals and their families during a challenging time.
About the Role:- Provide direct patient care under the supervision of a Registered Nurse
- Follow the plan of care, provide treatments, and work collaboratively with the healthcare team to achieve positive patient outcomes
- Assess and provide patient and family/caregiver education and information pertinent to diagnosis and individualized plan of care
- Participate in coordination of hospice services, reporting identified needs to the interdisciplinary group and Clinical Manager
- Graduate of an accredited practical nurse or vocational nursing program
- One to two years of nursing experience, preferably in community health/hospice or medical/surgical
- Currently licensed as an LPN/LVN in the State(s) of planned practice
- Complies with accepted professional standards and practice
- Good verbal and written communication, and organization skills
