Job Title: Licensed Practical Nurse LPN Hospice Full Time

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Signature Healthcare at Home. As a LPN, you will play a vital role in providing high-quality patient care to individuals in need of hospice services.

Job Summary:

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a Registered Nurse. This includes following the plan of care, providing treatments, and working collaboratively with the interdisciplinary team to achieve positive patient outcomes.

Responsibilities:
  • Provide direct patient care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care.
  • Implement current nursing practice following a comprehensive assessment and the plan of care.
  • Provide accurate and timely documentation of patient services to reflect the plan of care.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and individualized plan of care.
  • Participate in coordination of hospice services, appropriately reporting identified needs to the interdisciplinary group and Clinical Manager.
Requirements:
  • Graduate of an accredited practical nurse or vocational nursing program.
  • One (1) to two (2) years nursing experience preferred, with community health/hospice or medical/surgical experience being a plus.
  • Currently licensed as an LPN/LVN in the State(s) of planned practice.
  • Complies with accepted professional standards and practice.
  • Demonstrates good verbal and written communication, and organization skills.
