We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team at Mercy. As an LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

Key Responsibilities
  • Assess, plan, implement, and evaluate patient care needs and outcomes
  • Provide skilled nursing services to patients in their homes under the direction and supervision of a Registered Nurse
  • Collaborate with interdisciplinary teams to develop and implement patient care plans
  • Conduct thorough patient assessments and develop individualized care plans
  • Administer medications and treatments as prescribed by physicians
  • Monitor patient progress and adjust care plans as needed
Requirements
  • Graduate of an accredited practical nursing program
  • Certification/Registration: Understands Hospice philosophy
  • Complies with Unity Health Hospice Health Status requirements
  • Ability to work independently and as part of an interdisciplinary team
  • Excellent communication and interpersonal skills
Preferred Qualifications
  • One year of experience in a related field
