Job Summary:

The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of a registered nurse. This includes following the plan of care, providing treatments, and working collaboratively with the healthcare team to achieve positive patient outcomes.

Responsibilities:
  • Provide direct patient care as defined in the State Nurse Practice Act
  • Implement current nursing practice following a comprehensive assessment and the plan of care
  • Provide accurate and timely documentation of patient services to reflect the plan of care
  • Evaluate patient and family/caregiver education and information pertinent to diagnosis and individualized plan of care
  • Use and prepare equipment and supplies effectively and efficiently
  • Provide appropriate pain/symptom management and evaluate patient response to treatments/medications
  • Participate in personal, professional growth and development, and organization's quality assessment performance improvement teams and activities
Requirements:
  • Graduate of an accredited practical nurse or vocational nursing program
  • One (1) to two (2) years nursing experience preferred, with community health or medical/surgical experience being a plus
  • Currently licensed as an LPN/LVN in the State(s) of planned practice
  • Complies with accepted professional standards and practice
  • Demonstrates good verbal and written communication, and organization skills
