Job Summary

We are seeking a highly skilled and compassionate Patient Experience Specialist to join our team at CHAS Health. As a key member of our staff, you will play a vital role in improving the overall health of our communities by providing exceptional customer service, managing health records, and coordinating referrals.

Key Responsibilities
  • Customer Service: Provide top-notch customer service as the first point of contact for CHAS Health, ensuring that patients receive accurate and timely information about our services, locations, and providers.
  • Health Record Management: Manage and maintain accurate and up-to-date electronic health records, ensuring that patient information is secure and compliant with HIPAA regulations.
  • Referral Coordination: Coordinate referrals to specialists, ensuring that patients receive timely and appropriate care, and that referrals are processed efficiently and effectively.
  • Documentation: Document patient cases, health information, and referral requests accurately and thoroughly, ensuring that patient records are complete and up-to-date.
Requirements
  • Education: High school diploma or equivalent preferred, with some college preferred.
  • Experience: Experience with databases, electronic health records, and release of information preferred.
  • Skills: Excellent customer service, oral and written communication skills, attention to detail, and ability to work well in a team environment.
Work Environment

This role requires sitting at a workstation for most of the day, with occasional movement around the facility. Ability to lift up to 35 pounds is also required.
