Job Summary:
The Clinical Operations Manager is a key member of our HarmonyCares team, responsible for overseeing the coordination and delivery of high-quality hospice care services. This role requires a strong clinical background, excellent leadership skills, and the ability to work collaboratively with our interdisciplinary team.
Key Responsibilities:
- Monitor and ensure seamless coordination of referrals to our hospice program, ensuring timely follow-through and communication with our team.
- Assist in the coordination and assignment of newly admitted hospice patients to the appropriate team and disciplines, ensuring a smooth transition and effective care delivery.
- Work collaboratively to ensure all admission documentation is completed within 24 hours and disseminated among team members, promoting efficient communication and care coordination.
- Ensure effective communication of patient admissions and changes in Plan of Care are relayed to each member of the Interdisciplinary Team and Primary Care Physician, fostering a culture of transparency and collaboration.
- Serve as a member of the IDT committee, participating in all IDT meetings to discuss admissions, certifications, recertifications, and medical management of patients.
- Consult with the Hospice Medical Director on matters related to patient care services, ensuring alignment with our organization's mission and values.
- Participate in routine visits and admissions of patients during times of high census or low staff availability, demonstrating flexibility and a commitment to delivering exceptional care.
- Select and maintain a qualified, well-organized staff to provide care for the needs of our patients, ensuring a high level of quality and compassion.
- Be accountable and supervise the IDT team, including RNs, LPNs, Medical Social Workers, Chaplains, and Hospice Aides, promoting a culture of accountability and excellence.
- Establish staffing patterns that reflect the quality and quantity of various professional personnel necessary to plan, provide, and supervise care rendered to patients and families.
- Establish methods for coordination of care by all disciplines, ensuring a seamless and effective care delivery process.
- Be responsible for ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care, aligning with our organization's commitment to delivering person-centered care.
Requirements:
- Current registration as a nurse in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
- Two years or more of supervisory and/or administrative experience managing clinical teams.
- Five or more years of nursing experience.
- Maintain a valid driver's license and good driving record.
- Demonstrate a broad base knowledge of community health nursing and competent clinical skills.
- Ability to be available in person or telephonically to assist with after-hours administrative on-call responsibilities.
- Proficient computer skills, including but not limited to, use of Word, Excel, report software such as HomeCare HomeBase, and Outlook.
- Excellent verbal and written communication skills with demonstrated leadership ability.
- Excellent organizational skills.
- Ability to travel (up to 25% or as needed).
Preferred Qualifications:
- Certified Hospice and Palliative Nursing.
- Previous experience with Electronic Medical Records.
- Hospice experience.
- Previous experience in handling new hospice referrals from first contact through admission.
- A working knowledge of Community Health Accreditation Program (CHAP) on Accreditation, Quality Assurance, and Hospice Knowledge.
